Cisapride, an inorganic compound, is a benzoamide derivative. It is a new type of total gastrointestinal motility drug.
Purpose
1. Gastrointestinal motility drugs can strengthen and coordinate gastrointestinal movement, prevent food retention and reflux, so they have antiemetic effect.
2. There is also a strong stabilizing effect, which can be used as the treatment of psychosis. There is no blocking of dopamine receptor and direct stimulation of cholinergic receptor, so there is no corresponding side effect.
3. For gastroparesis, gastroesophageal reflux, upper gastrointestinal discomfort, chronic constipation, pseudo intestinal obstruction, etc.
Pharmacology and Toxicology
1. Animals:
(1) In the separated organs, it can prevent gastric relaxation, accelerate gastric peristalsis, and coordinate the movement of gastric antrum duodenum, small intestine and large intestine.
(2) In dogs, it can enhance the digestive activity of antrum duodenum, coordinate and strengthen gastric emptying, increase the peristalsis of small intestine and large intestine and shorten the time of intestinal movement, but it does not affect gastric secretion.
(3) The main mechanism is to promote the physiological release of acetylcholine in intestinal myenteric plexus.
(4) It does not stimulate muscarinic and nicotinic receptors, nor inhibit the activity of acetylcholinesterase.
(5) There was no dopamine receptor blocking effect at the treatment dose.
(6) It is mainly distributed in stomach and intestine.
2. Human beings: (1) Gastrointestinal motility effect: ① Esophagus: enhance esophageal peristalsis and lower esophageal sphincter tension; Prevent gastric contents from flowing back into the esophagus and improve the clearance rate of the esophagus. ② Stomach: increase the contractility of stomach and duodenum and the coordination of antrum duodenum; Reduce duodenogastric reflux; Improve gastric and duodenal emptying. ③ Intestines: strengthen the movement of intestines and promote the transport of small intestine and large intestine. 3. Other effects: (1) Due to the lack of cholinergic effect, it did not increase the amount of basic and pentagastrin induced gastric acid secretion. (2) Due to the relative lack of dopamine antagonistic properties, there is little effect on plasma prolactin levels. (3) It did not affect psychomotor function, blood pressure, respiratory rate, body temperature, weight and anticoagulant function. (4) The pharmacological action begins 30 ~ 60 minutes after oral administration.
